Harmony’s Continuum of Care Includes Memory Care for Residents with Dementia

Harmony’s Continuum of Care offers residents consistency with the same Executive Director, Dining, Programming, Billing and compassionate Care associates regardless of their current care level, whether it be Independent Living, Assisted Living, or Memory Care.

Harmony’s licensed health care professionals will conduct an assessment to create a personalized care plan for you or your loved one to ensure quality of life and maximum independence. You can have peace of mind knowing that your loved one’s needs are being met, including assistance with medications, help with scheduling health care appointments, and even a friendly escort to meals and activities.

Medication Management

According to the CDC, about 350,000 patients need to be hospitalized each year to due to adverse drug events. Seniors are especially prone to this because they are taking more medications.

Harmony offers medication administration through licensed healthcare associates. Additionally, associates help residents to organize and manage refills and renewals, and ensure that all medications are reviewed for adverse drug effects.

Memory Care and the Best Friends Approach™

Harmony utilizes the Best Friends Approach™ in its Memory Care program. This approach is “grounded in the understanding that relationships are supremely important in dementia care and should reflect the essential elements of friendship, respect, empathy, support, trust and humor,” according to the Best Friends Health Profession Press.

“The Best Friends Approach™ promotes the dignity and individual rights of people with memory loss through knowing the history and preferences of each resident, communicating skillfully, and providing a supportive environment and meaningful engagement.”

Harmony’s Regional Director of Clinical Services Terri O’Neal said, “Harmony has one of the best Best Friends Programs for Memory Care residents. It ensures that they are engaged in daily, mindful activities with one and another and staff members. The staff is so caring and attentive to the residents making activities of daily living (ADLs) enjoyable and person-centered.

O’Neal also pointed out that Harmony’s nursing department meets daily to review the residents’ care.

“The Memory Care is a beautiful secured area with caring staff. The nursing department has a daily meeting to review the residents’ care, needs and changes of condition,” she said.

Harmony’s philosophy of caring for patients in its Memory Care program centers on companionship and personal care.

“Each individual resident is receiving very specialized, very customized care,” said Allison Waters, Harmony’s Marketing Account Manager.

Upcoming Health Clinic

All Harmony residents will have the opportunity to attend a health clinic on Thursday, Feb.  18 from 1:00 to 3:00 p.m.  A registered nurse will administer this free checkup and the following services:

  • Free wellness checkup
  • Blood pressure test
  • Health screening
  • Tips to lower blood pressure
